Assuming that nanotech is how magic works, then I don't see why the answer couldn't be "Yes!". The Einstein-rosen bridge may only be openable to a couple microns, so the nanos may have to disassemble you on one side of the bridge, and reassemble you on the other. Maybe.

And I will take a look at Karl Schroeder, thanks. And likewise, Elizabeth Bear's always a good recommendation.
